Criterion 2.5.1 MAPPING AND INITIAL INTERVENTIONS

 Standard 2.5 Strengthening SME services with PIS PK.

The implementation of UKM services is strengthened by PIS PK in an effort to create healthy families and healthy communities through community organization with the formation of community resource health efforts (UKBM) and healthy arrangements which are a form of implementation of the Healthy Living Community Movement (Germas).

a. Criterion 2.5.1 

The UKM person in charge, service coordinator, and UKM activity implementer together with the family development team carry out health mapping and interventions based on family problems according to the agreed schedule.

1) Main Thoughts:

a) Family visit activities carried out by the family development team are used to convey information and educational communication to families as initial intervention and are documented.

b) Documentation of the results of family visits is carried out by entering them in the healthy family application and/or in the healthy family profile (Prokesga).

c) Documentation of the results of family visits is carried out by entering the healthy family application and/or family health profile (prokesga).

d) Documentation of the results of the visit can include the results of initial intervention and the results of further intervention.

e) Documentation of the results of the initial visit and the results of the intervention (update) is carried out by the Puskesmas PIS-PK data management team.

f) The family development team delivers information and reports on the results of family visits and coordinates with the UKM person in charge and the service coordinator and UKM activity implementer so that further analysis and intervention can be carried out.

g) The family guidance team is the health workers at the Puskesmas formed by the head of the Puskesmas through a decree from the head of the Puskesmas.

h) UKM activities through PIS-PK as a form of intervention are carried out in accordance with the schedule agreed with the target community.

2) Assessment Elements:

a) A Family Development Team and a PIS-PK data management team were formed with clear job descriptions (R).

b) The family development team carries out family visits and initial interventions that have been planned through the preparation process and documenting these activities (D, W).

c) The family development team calculates the healthy family index (IKS) at the family, RT, RW, village/kelurahan, and Puskesmas levels manually or electronically (with the Healthy Family Application) (D).

d) The family guidance team conveys information about health problems to the head of the Community Health Center, the person in charge of UKM, the service coordinator, and the implementer of UKM activities to jointly analyze the results of the family visit and communicate with the person in charge of quality (D,W)

e) The family development team together with the UKM person in charge, service coordinator and UKM activity implementer arrange further interventions for the family according to health problems at the family level (D, W).

f) The person responsible for UKM coordinates the implementation of further interventions together with related parties (D, W).
