Criterion 1.3.4 COMPLETENESS OF EMPLOYEE DATA

 Criterion 1.3.4

Every employee has complete and up-to-date personnel documents.

1) Main Thoughts:

a) The Puskesmas is obliged to provide personnel documents, both in printed and digital form, for each employee who works at the Puskesmas as proof that the employee who works meets the specified requirements and that development efforts are made to meet these requirements. These personnel documents are managed in accordance with established mechanisms that can guarantee their completeness and up-to-dateness.

b) Health workers who work at Community Health Centers must have a registration certificate (STR) and/or practice permit (SIP) in accordance with statutory provisions.

c) The personnel document for each employee contains, among other things:

(1) proof of education (diploma),

(2) proof of valid registration certificate (STR),

(3) proof of a valid practice permit (SIP),

(4) description of employee duties and/or details of clinical authority of health workers,

(5) proof of training certificate,

(6) proof of work experience if required,

(7) employee performance assessment results,

(8) evidence of development/training needs,

(9) evidence of evaluation of the implementation of training results, and

(10) proof of implementation of orientation.

2) Assessment Elements:

a) Complete and up-to-date contents of personnel documents are determined and available for each employee working at the Pukesmas, and maintained in accordance with established procedures (R,D,O,W).

b) Periodic evaluation and follow-up is carried out on the completeness and updating of personnel documents (D,W).
