Criterion 2.8.4
Performance assessments on the delivery of UKM services are carried out periodically to show accountability in the management of UKM services.
1) Main Thoughts:
a) The Head of the Community Health Center, person in charge of UKM, service coordinator and UKM activity implementer are responsible for cultivating continuous performance improvement, consistent with the vision, mission and goals of the Community Health Center.
b) The Head of the Community Health Center together with the UKM person in charge, service coordinator and UKM activity implementer determine policies and procedures for assessing the performance of UKM services
c) The Head of the Community Health Center together with the person in charge of UKM needs to periodically assess the performance of UKM services.
d) The performance assessment is intended to demonstrate accountability in the management and implementation of UKM Community Health Centers and to make improvements if the results of the performance assessment do not reach the expected targets.
e) The assessment is carried out in a meeting of the head of the Puskesmas together with the person in charge of the Puskesmas UKM, service coordinator and UKM activity implementer.
2) Assessment Elements:
a) The Head of the Community Health Center, the person in charge of UKM, the service coordinator and the implementer of UKM activities conduct performance assessment discussions at least twice a year (R, D, W).
b) Prepare a follow-up plan for the results of the discussion on the UKM service performance assessment (D,W).
c) The results of the performance assessment are reported to the district/city regional health office (D).
d) There is evidence of feedback from the district/city regional health office on the report on the results of the UKM service performance assessment (D).
e) The results of feedback from the district/city regional health service are followed up. (D).
