 Criterion 1.2.4

Puskesmas guarantees the availability of data and information through the implementation of the Puskesmas information system.

3) Main Thoughts:

a) In an effort to improve health status in its working area, the Community Health Center provides data and information as material for decision making in accordance with community needs, as well as decision making at the policy level at the district/city regional health service, including conveying information to the community and related parties.

b) The availability of data and information will make it easier for the quality team, those in charge of efforts, service coordinators, and each activity implementer, both UKM and UKP, laboratories and pharmaceuticals, in planning, implementing, monitoring and evaluating the success of quality improvement activities. and safety of service users.

c) Implementation of the Puskesmas information system in accordance with statutory provisions.

d) The data and information includes at least basic data and program data as well as other data and information determined by the district/city regional health service, provincial health service and the Ministry of Health.

e) Basic data consists of the identity of the Puskesmas, the working area of the Puskesmas, the resources of the Puskesmas, and the program targets of the Puskesmas. Then, program data includes data on essential UKM, development UKM, UKP, and other programs, namely Puskesmas management, pharmaceutical services, public health nursing services, laboratory services, and family visits during PIS-PK activities.

f) Collection, storage, analysis and reporting of data entered into the information system is carried out in accordance with a predetermined periodization.

g) Distribution of information, both internally and externally, is carried out in accordance with provisions, including access to data and information must consider aspects of information confidentiality and the interests of data users in accordance with statutory provisions.

h) The Puskesmas information system can be organized electronically and/or non-electronically, and needs to be supervised/monitored and evaluated periodically.

2) Assessment Elements:

a) Collecting, storing, analyzing data and reporting and distributing information is carried out in accordance with the provisions of laws and regulations related to the Puskesmas information system (R,D,W).

b) Evaluation and follow-up are carried out on the implementation of the Puskesmas information system periodically (D,W).

c) There is information on the achievement of Puskesmas performance through the Puskesmas information system (D,O).
